Do you keep track of your own medical records?
Research has shown that patients who self-advocate by personally managing their medical records have also enjoyed better health. Even though there are many benefits to recordkeeping, it can take a lot of time, energy, advocacy, and persistence. On top of that, it can require technical knowledge or strong computer skills.
Because of this, we’ve created templates to help you measure your progress and stand up for your rights as a patient with PF.
We recommend reaching out to friends, family, or caregivers to help you print and fill these forms out. It’s a worthwhile investment for your long-term health and quality of life, but it’s very difficult to do on your own.
To begin your recordkeeping efforts, click here. Then print the template & fill it out from home. The photo below shows a preview of the template titled “Weekly Update.” You can make multiple copies of it and fill one out each week. Try it for a couple weeks and let us know what you think!
